60 minute psychotherapy

2 appointment series with the following breakdown:

• Appointment 1: Computerized Testing: $79 fee (No insurance billing)
• Appointment 1: 60 minute initial diagnostic assessment: $185 (May be billed to insurance)
• Appointment 2: 30 minute follow up appointment: $100 (May be billed to insurance) Results of testing provided, explained and referral to psychiatrist for medication management

Clients with health insurance are typically only out of pocket $79

Forms provided to client for FMLA regarding MENTAL HEALTH. No insurance billing

Therapist to attend meeting related to educational needs including 504 and ARD meetings. No insurance billing

The following requirements must be met to be considered for our office to sign forms for an emotional support animal: 

Animal Requirements

1. Type of animal: Dog or Cat Only (Both must be able to be in public with you at all times)
2. Animal must be 4 years or older and have proof of DOB
3. Animal must be up to date on all shots/vet appointments and show consistency over the past 4 years. 
4. Affidavit stating no bite history and release of liability of therapist.

Patient/Client Requirements

1. Client must have been in therapy WITH our office for 8 months or more with no gaps in weekly sessions.
2. Client must have stable housing. This must include either purchase of the residence or 12 months or more rental lease. (No temporary shelters, homelessness, or other temp housing settings)
3. Diagnosis of client must directly related to the need for emotional support animal.
4. Other requirements as deemed necessary to make an appropriate recommendation by therapist.
